Community Service Hours Log
A free volunteer hours form with a supervisor signature on every row
A community service hours log is the sheet a volunteer fills in each time they serve: the date, what they did, when they started and finished, the hours, and a supervisor's signature. Schools, clubs, and courts usually accept a log like this as long as every row is signed. Download it below as a PDF to print, or as Word or Excel to add your organization's name first.

How to fill in a volunteer hours form
- 1
Fill in the top once
Your name, the organization, and a supervisor name and phone number someone can call to check the hours.
- 2
One row per visit
Date, the activity ("sorted cans at the food pantry"), time in, and time out.
- 3
Work out the hours
Time out minus time in, in hours. 9:00 to 11:30 is 2.5 hours. Leave breaks out.
- 4
Get the row signed the same day
A signature added weeks later is the one that gets questioned. Ask before you leave.
- 5
Total it before you hand it in
Add the Hours column at the bottom and keep a photo of every page.
What makes a log get accepted
- Every row has a date, a specific activity, and a signature
- The supervisor is a staff member or volunteer coordinator, not a family member
- The organization is the kind your school or program counts. Many exclude work for a family business or for pay, so check first
- Hours add up: time out minus time in matches the Hours column
- If your school or court has its own form, use theirs and keep this log as your backup record
For organizers: track hours from the sign-up
Set up the shifts: volunteer sign up sheet.
If you run the volunteer shifts, the sign up sheet already records who claimed which shift and when it ran. Export it after the event and you have the hours for every volunteer without collecting paper logs.
SignupKit is free for 5 sheets with 50 sign-ups each and 50 reminder emails a month, with no ads and no account for the people signing up.
